Include the government notice or receipt where possible so a coordinator can align with any response deadline. Do not upload passports of overseas participants — the laboratory and consular post will request what they need directly.

• I understand that immigration DNA testing is not required in every immigration matter and should follow the instructions of the applicable government authority.
• I understand that the official immigration DNA case must be initiated directly between the petitioner or authorized customer and an AABB-accredited relationship-testing laboratory.
• I understand that NorthStar Lab Services does not independently select the accredited laboratory or initiate the official laboratory case on my behalf.
• I understand that submitting this form is a preliminary intake and collection-coordination request only.
• I understand that submitting this form does not establish the laboratory case or confirm a collection appointment.
• I understand that NorthStar performs or coordinates authorized specimen collection but does not perform the laboratory DNA analysis.
• I understand that collection cannot proceed until the accredited laboratory establishes the case and provides or approves the required collection process.
• I understand that overseas collection must be coordinated through the accredited laboratory and the designated U.S. embassy, consulate, or other approved location.
• I will not independently send or deliver DNA collection materials to an overseas participant.
• I understand that the accredited laboratory controls the release of results according to the applicable government instructions.
• I understand that DNA testing addresses only the claimed biological relationship and does not guarantee approval of a visa, petition, passport, citizenship claim, or other immigration benefit.
• I confirm that the information provided is accurate to the best of my knowledge.
• I authorize NorthStar to communicate with the listed participants, attorney or representative, selected accredited laboratory, and authorized collection locations as reasonably necessary to review and coordinate the Michigan collection.
• I authorize NorthStar to securely share relevant submitted information and documents with the selected AABB-accredited laboratory when needed and authorized.
• I have read and agree to the applicable Terms of Service, Privacy Policy, cancellation policy, document-upload terms, and DNA testing service terms.
• I understand that typing my name constitutes my electronic signature for this submission.
